- Definition: Environmental pollution includes harmful substances or forms of energy (e.g., air, water, soil pollution, as well as noise, artificial light, and electromagnetic fields).
- Relevance to health: These exposures are linked to cardiovascular disease, respiratory disease, cancer, sleep disruptions, and cognitive effects.
- Focus: Not only obvious high-level exposure (e.g., wildfire events) but also chronic low-level exposure (e.g., living near a highway, cooking without ventilation).
- No randomized trials: It is unethical and impractical to randomize people to decades of different exposures.
- Small effect sizes: Unlike smoking (hazard ratio >10), environmental risks often show hazard ratios around 1.3 – too small for clear causal inference from observational data.

- Correlation vs. causation: Those near highways are also exposed to more noise and light – disentangling individual effects is nearly impossible.
- Multifactorial outcomes: Diseases like hypertension are influenced by genetics, diet, sleep, exercise – pollution is just one of many contributors.
- Health hierarchy: Sleep, exercise, and nutrition are far more impactful for most people than environmental exposures.
- Avoid over-reaction: Spending $2,000 on an air filter while sleeping 5 hours a night is a misallocation of resources.
- Focus on high-return actions: Rather than trying to eliminate all exposures, identify the 20% of efforts that yield 80% of benefits (e.g., a quality air purifier in highly polluted areas).
